To participate in high school athletics in South Carolina, several forms are legally required.  These include a physical examination form completed by a medical professional, a government-issued birth certificate, and various permission forms to be completed by a parent or guardian.  These forms MUST be completed prior to trying out for or participating in any sport. The forms are required to be completed again for each school year, but are good for all sports during that year. 

These forms are now online and may be accessed on BigTeams (formerly PlanetHS).  Students should create an account and sign in.  Detailed instructions are found in the link to the left.
